New Patient Forms
We have organized all the new patients forms as one convenient package below. The forms can be completed and printed on-line. Please bring a completed copy of the forms with you on your first office visit. Please note that the link will open in a new window.

Your First Visit
When visting us for the first time, please have the following information handy:
- Your dental care insurance ID card, if you are currently subscribing to any dental insurance.
- A list of all prescription or non-prescription medications you have recently taken or are taking now.
During your visit, we will complete an in depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ and other signs of periodontal disease.
Please Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.
For your convenience, we have our new patient forms available to you here, which you can complete online, print and sign it, to bring in when you have your appointment. For your convenience, we recommend printing and completing these forms at home.
Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly email or fax them to you.
